upped or up'd means upgrade or upgraded. Although you can't upgrade unique items on non-ladder, the recipe for upgrading unique items on ladder is
Lem + Ko + P diamond + unique item in the Cube

actually, tjhats the recipe for armor... the recipe for weapons is different. but yeah

im prety sure it pul lum pamythest for weps to up them from exceptional to elite

theres also different recipes for upgrading to exceptional on both weapons and armor.

Armor includes, gloves, boots, helms, and belts

Normal to Exceptional armor: Tal + Shael + Perfect Diamond
Exceptional to Elite armor: Ko + Lem + Perfect Diamond
Normal to Exceptional Weapon: Ral + Sol + Perfect Emerald
Exceptional to Elite Weapon: Lum + Pul + Perfect Emerald

that covers the formulas for all unique items. u can also up rares
