To get the keys, you need to kill each of the three keyholders (The Countess, The Summoner and Nithalak) and make them drop their key. When you got all three keys, put them in your cube and go to Act 5 and open a Pandemonium Portal, there is three different portals leading to these three places;

does that mean, when you open the first portal, once you go in there is 3 other portals?, ive never done this before

No you need 9 keys if you get a different portal each time. It's random on the portal selection, just trade away for the one you want with the organ you have duplicates of. Make sure not to make 2 portals in the same game, they first one would over lap the second one.

actually, its best to make all 3mini portals in 1game, that way u are sure to get the 3 different organs. just dont make the portals right on top of each other and they wont overlap ive done this lots of times
