This isnt a guide but it will get you there. You have a few choices but here is mine, although I prefer frenzy barb for pvm. You dont need a shield for pvm its pointless.
2 x ebotd. There are other options but you will be alot happier with ebotd for pvm.
Fortitude is my armor of choice but enigma is a good second because of tele.
Dungos/string of ears. I always love dungos they are the best for most everything barb related but string of ears is a great alternative if need be..
Arreats
Dracs/steelrends Either way u cant go wrong. dracs for lifetap and rends for more damage. Although since you are going dual ebotd then I'd use rends. You wont need the lifetap and the rends will do alot more raw ramage which is good for pvm
Angelic jewelry you will need the ar and there is no better place to get it. You can always swap 1 ring for a raven. This is really your only good option here.

Warcry sticks on the switch.
Max ww, shout, Battle order. 1point in resis. 1 point in speed. Max whatever weapon you are using. put rest in iron skin.
Like I said I;m not writing a guide so I didnt go into every detail but this setup will do well in pvm. Frenzy barb is always better pvm though. Frenzy barb pretty much never dies unless you get hit with IM.
