@Halicarnasos, do you want ES or full VIT build? ES is better but you require godly items. Let me know and I'll write something ASAP. As in for your first questions:
Spirit > 20/-20 Monarch any day
Eschuta v. HOTO:
Depends on the rest of your gear and resists, however as a general rule, use Eschuta against Barbs, Smiters and melee damage based chars and HOTO v. Elemental dam dealers (i.e. sorcs and trappers).
