Armor - Chains of Honor
Weapon - Ethereal BotD Thunder Maul or Cranium Basher (!)
Belt - Verdungo's
Gloves - Laying of Hands
Boots - Gore Riders
Rings - Two Bul-Khathos
Amulet - Highlord's Wrath
Weapon2 - +3 to Warcries weapon
Weapon3 - See weapon 2

PvP:
Helm - 'Cham' Arreats or Ravenfrost
Armor - Enigma
Rings - Two Angelic Rings, Ravenfrost and Angelic Ring if you aren't using 'Cham' Arreat's
Amulet - Angelic Mantle
Gloves - Steelrends***
...and everything else is the same as the PvM setup.
