If you're meaning The Cave is in Blood Moor it's because the Den Of Evile quest is already done.

no its not because of that i have done many games with it done...its just at some maps it shows up like that and most of the time if u go to cold plains it shows the den of evil there...so its reversed

if the maker of the game has den of evil done it will change into "the cave" and there are more than 1 "caves" in act1.. if thats not what ur pointin out then what is =/
