I've been running D2 in window mode but everytime I click outside of the window, it minimizes and it's annoying because this happens while I'm tele-ing. Is there a way to keep the window mode on top so it doesn't minimize? Thanks.
(Edit: I did a search and found that D2loader has something that will fix this however, I'd rather not take any chances with D2loader.)

add the -nohide in shortcut

This is mine, make sure you have it like this.
"C:\Program Files\Diablo II\D2Loader-1.11b.exe" -w -nohide -title "D2" -direct -txt -sleepy

I think you both think that I'm using D2loader, which I am not. Is D2loader the only way to "fix" this window issue?

Yes, you need d2loader for this command thing.
