1 Perfect Shaefer off an act 5 witch in Baal's throne room
1 eth guardian angle off a flayer
1 120 defense shako off a carver
and 1 dracul's grasp off lister but i know he usually drops good

I got Tal's ammy from normal Meph. (I haven't investigated as to whether this is actually possible or not.) I found it again with the same character from a random Flayer in act 3 hell.

It took me a good half hour to find a decent avatar...goddam Final Fantasy

if meph's above lvl 26 it is possible

funny the ilvl in tals is 26 but clvl is 67

I found an Oculus in baals worldstone lv.2 or 3 on nm difficulty from some unique dude with only 69 mf.

When I killed Baal Hell no mf and i got a shafstop, occulus, arreatsface, and immortal king armor. and two other rares.

i found Pierre Tombale Couant Partizan from ball, i know its not a monster but i got ginthers rift from gambling(i made charms that decresed prices for items so i made money from gambling and just kept buying untill i got a unique).
