Upgrading items..........

i've heard some small amounts about upgrading items. could i get the actual facts on that? i have absolutely no clue what it entails, so any information would be a big help. also, if this is new to 1.10, then i have an excuse for being dumb to it. i've just started up diablo again, and have been nearly overwhelmed with the changes in 1.10 (well, not really overwhelmed, but there's a lot to learn again). again, i'd appreciate any help you could give.
